ABSTRACT

Objective To improve the analysis efficiency and interactive experience of the Military Electronic Health Records System(MEHRS)and to realize quick response of ad-hoc queries and statistics in the MEHRS with big data columnar storage and processing technologies.Methods We carried out requirement analysis and functional design of the ad-hoc queries and statistics subsystem of the MEHRS,proposed a three-tier architecture which included the archive storage layer,statistical pretreatment layer and statistical application layer.After the selection and evaluation of big data processing technologies,CarbonData columnar storage was used to store preprocessed data and executed statistics with Spark SQL on the basis of medical business data modeling and preprocessing.Results Five testing tasks were executed on two million archives in the following two subsystems:one with modeless and non-preprocessed MongoDB storage,the other with modeled and preprocessed CarbonData storage.The latter could finish these tasks within seconds and was dozens of times more efficient than the former statistically.Conclusion This study designs and implements a big data technology proposal that satisfies the quick response of ad-hoc queries and statistics in the MEHRS, providing powerful and flexible technical support for big data statistical analysis.

ABSTRACT

Objective To explore a construction method of semantic relations of the top-level ontology of military medicine in order to construct the top-level ontology semantic network.Methods The military medical corpus was selected,and the relationships between the concepts were extracted using text analysis.On the basis of inheriting the semantic relations of UMLS,the special semantic relations of military medicine were added.Results A method of establishing the semantic relations of the top-level ontology of military medicine was proposed,the applicability of which was verified by example of the branch of military medical equipment.Conclusion The proposed method is effective and feasible,which can provide important support to the establishment of top-level semantic networks of military medicine.

ABSTRACT

Objective To store and use the military electronic health record ( EHR) data effectively , and to improve the flexibility, scalability, and statistical query performance of data storage .Methods The military EHR data access patterns were analyzed,a military EHR storage model using MongoDB document database was designed ,and the storage model was optimized by storing the whole document and the reduced summary document separately .Results The storage model was used in the military EHR system , which can meet the demands of document query and statistical query at the same time . The optimized model improves the statistical query performance without losing scalability .Conclusion This model can meet the daily needs of the EHR system and is of practical application value.

ABSTRACT

<p><b>OBJECTIVE</b>To evaluate factors for predicting ventricular arrhythmia, the clinical effect of drugs on patients with catecholaminergic polymorphic ventricular tachycardia (CPVT), and their long-term outcomes.</p><p><b>METHODS</b>Six patients diagnosed with CPVT underwent a series of electrocardiograms and 24-hour Holter monitoring. β-blockers were recommended for all patients, while some patients were also prescribed propafenone and 1 patient underwent catheter-based renal sympathetic denervation (RDN). The characteristics of electrocardiogram, arrhythmia and long-term outcomes were monitored.</p><p><b>RESULTS</b>Syncope episodes did not occur any longer in 1 patient on β-blocker, but recurred in 3 other patients and 2 patients died (one due to his cessation of metoprolol for 3 months). Inverted and/or bifid T waves and abnormal U wave were observed in the precordial leads. T wave alternans was observed in 4 patients in the precordial leads. These abnormal electrocardiogram features disappeared or diminished with β-blocker treatment. All spontaneous episodes of ventricular tachycardia occurred prior to sinus tachycardia and frequent polymorphic premature ventricular contractions.</p><p><b>CONCLUSIONS</b>Bifid and/or inverted T waves, T wave alternans and abnormal U waves together with sinus tachycardia and frequent premature ventricular contractions are indicator for predicting ventricular arrhythmia and assessing the effect of β-blockers. Compliance with β-blocker treatment is a strong indicator of outcome.</p>

ABSTRACT

<p><b>OBJECTIVE</b>To describe the clinical features of 6 patients with catecholaminergic polymorphic ventricular tachycardia.</p><p><b>METHODS</b>Clinical data including signs and symptoms, electrocardiograms, Holter monitoring electrocardiograms and echocardiography was analyzed. Definite diagnosis was made based on the mutations of RYR2 and CASQ2.</p><p><b>RESULTS</b>From July 2002 to March 2010, 6 consecutive patients referred to our center because of syncope [4 males, mean age (13.0 ± 4.2) years] were diagnosed with CPVT by clinical evaluation and genetic testing. Their electrocardiograms showed T waves with notch or bimodal and tall U waves in right chest leads. There was no J wave, no ST-segment deviation, no prolongation or shortening of QT interval. We captured the so-called "bidirectional and(or) polymorphic ventricular tachycardia (bVT and pVT)" in 2 out of 6 patients by ECG, in 5 out of 6 patients by 24-hours Holter monitor, in 3 out of 6 patients by exercise test. All patients received β blockers and no syncope occurred during the 3 months follow-up after discharge from hospital.</p><p><b>CONCLUSIONS</b>CPVT is an inherited cardiac channelopathy characterized by syncope and(or) sudden death relatived to motion. The ECG shows T wave alteration and tall U wave in right chest leads. The mode of its onset is bVT and(or) pVT, and can be captured by Holter easily. β blocker is a safe and effective remedy for suppressing its attack.</p>

ABSTRACT

<p><b>OBJECTIVE</b>To investigate the method and clinical outcome of reconstructing large pulp defects of the fingertips with a homodigital neurovascular island flap.</p><p><b>METHODS</b>Form June 2007 to October 2009, 16 patients with the defects of pulp were repaired by the spiral flap that was a homodigital neurovascular island flap with a unique spiral advancement and transposition design allowed pulp reconstruction using sensate glabrous skin while restricting donor morbidity to the injured digit. There were 12 males and 4 females with an average age of 37 years ranging from 18 to 49 years. The defect were caused by machine crush injury 11 cases, pressure injury by heavy objects in 2 cases, crush injury by door of car 1 case, injury by saw in 2 cases. The defect was located in the index finger in 5 cases, the middle finger in 9 cases,the ring finger in 2 cases. All injuries had large pulp defects averaging 1.6 cm (long) x 1.2 cm (wide) to 2.5 cm (long) x 1.7 cm (wide). Short-term results for all patients were reviewed. Outcome measures included static 2-point discrimination, total active motion, and hypersensitivity or cold intolerance.</p><p><b>RESULTS</b>All flaps achieved primary healing with no complications. Sensory recovery was excellent with an average 2-point discrimination of 5.1 mm. All patients were followed-up for 8 to 20 months (averaged 12 months) with highly satisfactory with both aesthetic and functional outcome. There was no hypersensitivity or cold intolerance. According to the evaluation of total active motion (TAM) scales, the results were excellent in 7 cases, good in 8 cases (9 lesions) and fair in 1 case.</p><p><b>CONCLUSION</b>The spiral advancement-transposition flap is suitable for resurfacing large pulp defects with excellent short-term functional and aesthetic results and high patient satisfaction.</p>

ABSTRACT

Objective:To establish hospital infection data sharing and service platform for collecting and issuing the hospital infection data. Methods:The relational database was used to realize data storage and J2EE light structure was employed to enhance the development and the operating efficiency and flexibility of the system according to the principles of Web information system.Results and Conclusion:An open and sharing hospital infection data platform for data processing, retrieving and analyzing was established. The established hospital infection data set and surgery infection data set contained more than 20 000 records. The system provided an effective software platform support for the utilization of hospital infection data and the prevention and control of hospital infection. This article introduces key technologies of the design and realization of the system.

ABSTRACT

<p><b>OBJECTIVE</b>To analyze the application of vertebral pedicle screw fixation in the treatment of burst fracture of thoracolumbar vertebrae.</p><p><b>METHODS</b>A total of 48 cases (31 males and 17 females, aged from 18-72 years, mean: 41.3 years) with thoracolumbar vertebrae burst fracture were treated by pedicle screw system since January 2004. According to the AO classification of thoracolumbar vertebrae fracture, there are 36 cases of Type A, 9 of Type B and 3 of Type C.</p><p><b>RESULTS</b>All patients were followed up for 6-25 months (average 12 months), no secondary nerve root injury, spinal cord injury, loosening or breakage of pedicle screw were observed. The nerve function of 29 patients with cauda equina nerve injury was restored to different degrees. The vertebral body height returned to normal level and posterior process angle was rectified after operation.</p><p><b>CONCLUSIONS</b>The vertebral pedicle screw internal fixation was technologically applicable, which can efficiently reposition and stabilize the bursting fractured vertebrae, indirectly decompress canalis spinalis, maintain spine stability, scatter stress of screw system, reduce the risk of loosening or breakage of screw and loss of vertebral height, and prevent the formation of posterior convex after operation.</p>

ABSTRACT

Objective To explove the replantation and repairing methods of specified tissues and or- gans.Methods Seven cases of amputated external ear,2 cases of avulsed penis and testicle,1 amputated tongue and 1 amputated nose had been treated by microsurgery replantation,temporary ectopic implantation, and reconstruction by flap transfer.Results Those success in 5 cases of external ear replantation,2 earflap reconstruction with flap,and 2 replantation of penis and testicle,while failure in 1 case of external ear tempo- rary ectopic implantation,1 replantation of tongue and 1 nose.Conclusion For amputated special tissues and organs,shape and function of amputated tissues and organs can be well reestablished by microsurgery re- plantation.Skilled technique of microvascular anastomosis and satisfactory braking are most important to suc- cess.

ABSTRACT

<p><b>OBJECTIVE</b>To investigate the constituents of anti-inflammatory and hemostatic active sites of Mentha spicata.</p><p><b>METHOD</b>Chemical constituents were separated by solvent method and chromatography and identified by physicochemical properties and spectroscopic methods.</p><p><b>RESULT</b>The structures were identified as ursane I, 3-methoxy-4-methylbenzaldehyde II, veratric acid III, 5-hydroxy-3',4',6,7-tetramethoxyflavone IV, diosmetin(V), thymonin VI, daucosterol VII.</p><p><b>CONCLUSION</b>Among them, compounds I, II, III were first obtained from M. genus, IV, VI were separated from M. spicta for the first time.</p>
